What should I put for time off request?

Your form should, at the very least, include:

  • The employee’s name.
  • Date request is made.
  • Time-off requested (date range or hour range)
  • Reason for time off.
  • Employee’s signature.
  • Manager’s signature.
  • Approval/denial confirmation.
  • Date request approved or denied.
